Care Management Assessment And Consultation

In addition to workshops, seminars and training opportunities, the professionals at CC Consultants are available to provide the following services:

*Are you working with an older client who is reluctant to make decisions or seems to be in conflict with family members?

*Do you have concerns about the client's living  situation or the client's cognitive status?

 We can assist by providing an assessment and make recommendations for further support and interventions.

Assessments include, but are not limited to:

*Physical, emotional, psychological and cognitive abilities.

*Family  and additional support systems such as friends, neighbors, and faith communities.

*General financial and legal planning.

* Physical environment and living situation.

* Referrals to appropriate professional and care providers, including Aging Life Care Specialists.
